Medical Internship Requirements

EIU Requirements 

Each intern is required to complete the following requirements prior to starting their internship.  

  • Vaccinations
    • Measles, Mumps, Rubella (MMR)
    • Varicella (Chicken Pox) 
    • Hepatitis B 
    • Tuberculosis (TB) 
    • Tetanus, Diptheria, Pertussis (Tdap) 
    • Influenza (Flu)
    • Seasonal Influenza Shot
    • Pertussis Immunization
  • CPR/BLS Certification
  • Health Insurance
  • Professional Liability Insurance
  • Physical Examination
  • HIPAA Certification
  • OSHA Certification
  • Handbook Acknowledgement
  • Student Acknowledgement of Student Responsibilities
  • Student Acknowledgement of Internship Policies
  • Student Code of Conduct
  • Student Confidentiality Agreement
  • Student Social Media Policy
  • Background Check
  • 10-Panel Drug Screen
  • Completed Fingerprinting

 

For specific site requirements, refer to your contract and communication from your internship site.  

 

Documentation

All required documents should be uploaded to the EIU Medical Internship 25-26 Sharepoint folder. Failure to complete all tasks on time will delay the start of your internship.

 

Timeline for Submission of Requirements

EIU Requirements for All Students Wednesday, July 2, 2025
Flu Shot for All Students Thursday, October 2, 2025
Additional Site Requirements for Fall 2025 Placements Wednesday, July 2, 2025
Additional Site Requirements for Spring 2026 Placements Sunday, November 2, 2025

 

*Note: Please submit requirements as soon as possible to avoid delaying the start of your placement. Submitting requirements earlier is better. Some requirements, such as the Varicella vaccine, may require waiting a month between doses. Submitting requirements early allows time to ensure what you submit completely fulfills both EIU and site requirements.
