Facility Reservation Request Form

All facility requests must be submitted to the Program Director in the Campus Recreation Office in the Student Recreation Center at least TWO WEEKS prior to the date of the activity.

**Incomplete facility reservation requests will not be considered**

To request equipment with no facilities please to submit a facility reservation request.
